HSAs vs. HRAs: Which is Better?

I had a conference call this week with the commissioners of a county who are considering adopting Health Savings Accounts for their employees. They said their current broker has been discouraging them from doing so in favor of a Health Reimbursement Accounts instead. They wanted to pick the brain of someone who has no axe to grind.

I told them that HRAs are fine, but not as effective as HSAs in most situations. I explained that the American Academy of Actuaries (AAA) has long held that the economizing behaviors depend largely on to what extent employees view the money as their own. HSA funds clearly belong to the employee and to no one else. HRA funds are always the employer’s money and may be spent solely on services that are covered by the employer’s health plan.

Aetna Study Says CDHC is A-Okay

The study of two million Aetna members shows sustained savings over time and increased member engagement in their health. The study results show that members with a Health Savings Account (HSA) had more than 15 percent lower primary care physician use for non-routine visits, and more than 10 percent lower overall medical costs than members in a preferred provider organization (PPO) plan. The results also show that HSA members are more involved in their health care — they are two and a half times more likely to use online tools and three times more likely to take a health assessment than their PPO counterparts.

ObamaCare May Be Fatal to Your HSA

A recent analysis by Roy Ramthun concludes the new law will probably lead to major changes in how consumers can use Health Savings Account plans.

The new law limits these consumer-controlled accounts in two ways:  it restricts the types of health products you can purchase with your HSA money, and it doubles—to a whopping 20 percent—the tax penalty for withdrawing HSA funds to cover non-medical expenses.

But the worst news is the provision requiring all policies to cover at least 60 percent of the actuarial value of the benefits offered.  What’s the actuarial value?  Will contributions to HSAs be included in these actuarial-value calculations?  HHS Secretary Kathleen Sebelius will make that call.  And if she rules “no,” then high-deductible health plans including HSAs will no longer be viable… and you can kiss your plan good-bye.
